ENT causes of headache
1. Earache
This is a common sympton which may be caused by an otologic condition or to
Referred pain. Most otologic causes of earache can be easily diagnosed with good
History and examination. It is important to remember that cranial nerves 5,7,9 and 10
As well as C2/C3 contribute to innervation of the ear. Therefore, lesions of structures
Supplied by these nerves can result in referred pain. Common examples are dental
Causes, TMJ dysfunction, tonsillitis, cervical and latyngeal lesions. It is also
Worthwhile noting that some lesions such as small cancers of the deep lobe of parotid
Are not easily detectable, unless radiological investigations are carried out.
2. Otologic causes
Structures of the ear and mastoid are in close proximity to the middle and posterior
Cranial fossae. Otitis media and mastoiditis can result in intracanial complications.
Infection can spread intracranially, resulting in epidural abscess, meningitis, subdural
Abscess, temporal/cerebellar abscess, lateral sinus thrombosis and otitic
Hydrocephalus; all of which can cause headaches.
3. Rhinogenic causes
Nearly 50% of patients with blocked nose secondary to deviated nasal septum have
Associated recurring headaches. Possible mechanisms of headaches are referred pain,
Blockage of paranasal sinuses and the release of Substance P. Sinusitis not only can directly in headaches, it can also indirectly cause headaches via intra-cranial
Complications.
